Rally Mecical Information Form?

There used to be a rally medical information form.  Seems to me that generally, it would be good to get it out to rally attendees.   Also, I'd like a new copy, my old one is now unreadable. 

Anyone have a copy of this anymore? 

Tried to find it on FJrally.com, but did not see it.
